In accordance with the Law of the Republic of Uzbekistan "On appeals of legal entities and individuals":

The appeals must indicate the surname (name, patronymic), information about the citizen’s place of residence, the essence of the statement, proposal or complaint.

A citizen has the right to submit an application, proposal and complaint in person, as well as authorize his representative to do this or send an appeal through communication. In the interests of minors and incapacitated persons, a statement, proposal and complaint may be submitted by their representatives in the manner prescribed by law.
